The Heart Failure Clinic at East Jefferson General Hospital answers a need within our community for heart failure patients to have ready access to testing, evaluation and clinical support, including follow-up appointments after hospital admission or visit to the emergency department, and ongoing medical management.

The Heart Failure Clinic is dedicated to providing easy referral of heart failure patients for follow-up and/or continued heart failure management within 72 hours of hospital discharge and within 24-48 hours of discharge from an Emergency Department.
