Appointments

Please arrive 15 minutes prior to your appointment time and bring your insurance cards and medications with you. If you are unable to keep your appointment, please notify us as soon as possible.

Prescription Refills

Have your pharmacist call us at 504-454-7878 with your refill request. Please place refill requests at least three days in advance. Narcotics will not be refilled outside of regular business hours.

Referrals

When appropriate, our physicians will make a referral to a specialist who participates in your insurance plan. If your insurance company requires that we notify them before you see the specialist, please schedule your appointment at least one week in advance.

Medical Records Release

An authorization signed by the patient or guardian is required before medical records can be released. Provide us with the name and address of the requesting party, and we will forward the appropriate information for you.

Test Results

Results are mailed to the patient within two weeks of the test or can be obtained on our patient web portal, myejgh.org. If your test results require immediate action, you will be notified by phone.

Fees and Payments

Fees or co-payments are due at the time of service and may be paid by cash, check or credit card. We accept most major insurance plans including Medicare and Medicaid.
